Ambassador Sign-Up Form

By joining the VitaTech Healing Ambassador Program, you agree to promote VitaTech Healing products in a respectful, honest, and professional manner. As an ambassador, you will receive a 20% commission on each qualifying sale generated through your unique referral link. Commissions are paid out bi-weekly after a 30-day holding period to account for returns or cancellations. You are responsible for reporting your earnings for tax purposes, and VitaTech Healing will issue a 1099 form if your earnings exceed $600 in a calendar year (for U.S. residents). Any misleading claims, misuse of discount codes, or inappropriate representation of the brand may result in immediate removal from the program and forfeiture of commissions. VitaTech Healing reserves the right to update these terms or terminate the program at any time with reasonable notice provided. By submitting your application, you acknowledge that you have read, understood, and agreed to these terms. A signature will be required at the bottom of the form to confirm your agreement. This agreement shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law principles.
